Medicaid Hearing Aid Benefits at Ear Level Communications

Medicaid Hearing Aid Benefits are now at Ear Level Communications! This means that Ear Level Communications now accepts Medicaid patients. If you are currently enrolled in Medicaid and would like to use your hearing aid benefits, here is what we will need from you:

  • A copy of your current Medicaid ID card
  • A copy of a valid Texas Driver’s License or state-issued ID card
  • A signed Physician’s Exam Report (Form B.41). Click here to print the form
  • A copy of any supplemental plan ID cards (Evercare, AmeriGroup, Molina, etc.)

Important: Medicaid Hearing Aid Benefits Changed as of December 2, 2008

New Rules regarding Medicaid Hearing Aid Benefits went into effect in December. Perhaps the biggest change is:

  • Eligible Medicaid enrollees may now be eligible for two hearing aids.

Previous benefits limited eligible participants to one hearing aid once every six years. Now, eligible participants may be able to receive two hearing aids once every five years. To see if you may be eligible we need your current Medicaid ID number.
